Myeloid sarcoma consists of the extramedullary proliferation of blasts which include one or more myeloid lineages. It is an extremely rare neoplasm, with an incidence of 2 cases per million inhabitants in the adult population. This disease can precede, coexist, or occur after an initial diagnosis of acute myeloid leukemia, which the most frequent form. We present the case of a 30-year-old woman who had been diagnosed with extramedullary myeloid sarcoma in the ileum without blasts infiltration into the bone marrow, which was proven by biopsy. The disease started with abdominal pain in May 2022, after the diagnosis she had an ileum resection performed and started with chemotherapy. At the moment she’s in complete remission as shown by positron emission tomography and she’s on midostaurin maintenance. The approach of the current case of isolated myeloid sarcoma has great value for research, since in addition of being a pathology described in few case reports, the location of the sarcoma in the patient makes it an even more unusual condition, making its description a key piece for the evolution in medical knowledge.
