Coccidioidomycosis is an endemic disease in Sonora and should always be considered when facing a case of pneumonia accompanied by rash and hilar lymphadenopathy. Epidemiological data for this disease are unreliable due to deficiencies in case reporting. Diagnosis should be supported by serological studies; however, it is important to remember that a negative serological test does not rule out the disease, and if present, azoles are the treatment of choice.
